Veterans Affairs SDoH Screening Tool ACORN (Assessing Circumstances & Offering Resources for Needs)

The VA ACORN screening tool can be used to help identify social determinants of health (SDoH) that are impacting any patient. The comprehensive tool is likely too long to be utilized in its entirety for every patient encounter, but it does provide language for physicians to use to identify SDoH for a particular patient. If you are unfamiliar with SDoH, this is a nice place to start to get an idea about the kinds of issues to consider with your patients.■
